Susceptibility weighted imaging (SWI) serves as a cornerstone of our advanced Traumatic Brain Injury (TBI) protocols due to its superior sensitivity in detecting injuries that standard MRI sequences often miss. It is specifically designed to exploit magnetic susceptibility to identify microhemorrhages, diffuse axonal injury (DAI), and venous abnormalities.
The role of SWI in our diagnostic process includes:
- Detecting Subtle Lesions: SWI is 2–4 times more sensitive than conventional gradient-recalled echo (GRE) for identifying hemorrhagic shear injuries. It can reliably visualize punctate hemorrhages as small as 1–2 mm, providing objective evidence of trauma even when other scans appear normal.
- Identifying Diffuse Axonal Injury: By detecting the deoxyhemoglobin and hemosiderin deposits left by microbleeds, SWI highlights axonal shear injuries at the grey-white matter junction, corpus callosum, and brainstem.
- Long-Term Monitoring: Because SWI can identify chronic microhemorrhages years after the initial trauma, it is essential for the long-term management of TBI patients.
- Comprehensive Evaluation: We integrate SWI with Diffusion Tensor Imaging (DTI). While SWI identifies hemorrhagic markers, DTI maps white matter tract integrity. Together, they provide a complete picture of both vascular and structural brain damage.
