It is specifically designed to exploit magnetic susceptibility to identify microhemorrhages, diffuse axonal injury (DAI), and venous abnormalities.
The role of SWI in our diagnostic process includes:
- Detecting Subtle Lesions: SWI is 2–4 times more sensitive than conventional gradient-recalled echo (GRE) for identifying hemorrhagic shear injuries. It can reliably visualize punctate hemorrhages as small as 1–2 mm, providing objective evidence of trauma even when other scans appear normal.
- Identifying Diffuse Axonal Injury: By detecting the deoxyhemoglobin and hemosiderin deposits left by microbleeds, SWI highlights axonal shear injuries at the grey-white matter junction, corpus callosum, and brainstem.
- Long-Term Monitoring: Because SWI can identify chronic microhemorrhages years after the initial trauma, it is essential for the long-term management of TBI patients.
- Comprehensive Evaluation: We integrate SWI with Diffusion Tensor Imaging (DTI). While SWI identifies hemorrhagic markers, DTI maps white matter tract integrity. Together, they provide a complete picture of both vascular and structural brain damage.
