What are the 2026 Cms Rules for Prior Authorization Timeframes?

Starting in 2026, the Centers for Medicare & Medicaid Services (CMS) will enforce new mandates to significantly shorten the timeframes for radiology prior authorization. These updated rules apply specifically to Medicare Advantage plans and are designed to expedite access to critical imaging like MRI and CT scans.

The new binding obligations for insurers include:

  • Standard Prior Authorization Decisions: Must be rendered within 14 calendar days.
  • Expedited Prior Authorization Decisions: Must be rendered within 72 hours.

To meet these accelerated timelines, the rules require the implementation of electronic prior authorization and new interoperability standards. These changes aim to minimize manual processing and facilitate faster data exchange between providers and insurers.
